Job summary

CentraCare Health is seeking an LPN at Sauk Crossing in Sartell, MN. The LPN will provide patient rooming for clinic visits and communicate results to patients and providers, while assisting with daily clinic operations and documentation in the EMR.

Responsibilities include telephone screening, administering medications and immunizations under supervision, and recognizing urgent needs to seek help. Eligible candidates may receive a sign-on bonus and relocation assistance, plus tuition

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure as a Licensed Practical Nurse (LPN) in Minnesota.

Responsibilities

• Provide patient rooming for all types of clinic visits.
• Communicate results and other pertinent medical information to patients and providers.
• Assist in day-to-day clinic operations and with clinic procedures.
• Document procedures and treatments in the electronic medical record.
• Perform telephone screening and gather information for RNs or Providers.
• Administer medications and immunizations under direction of the RN or provider.
• Recognize abnormal or urgent needs and seek appropriate assistance.
