Ten Broeck Center for Rehabilitation & Nursing in Lake Katrine, NY, seeks Full-Time and Part-Time Licensed Practical Nurses (LPN) to join our 258-bed facility known for subacute and long-term care. We offer a sign-on bonus up to $7,500, new rates, and the most affordable health insurance plans.

Responsibilities include admissions, medication administration, and collaboration with an interdisciplinary team to ensure high-quality resident care.

Qualifications

• LPN licensed by the State.
• CPR Certification required.
• Two Years prior nursing experience preferred.

Responsibilities

• Admissions, Discharge and Transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Following infection control, emergencies, and other center policies.
• Collaborating with resident’s interdisciplinary team to provide the best outcomes.
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy.
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• Implements the patient/resident plan of care and evaluates the patient/resident response.
• Directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment setup, maintenance and use.
• Maintains confidentiality and patient/resident rights.
