Qualifications

• Valid and un-encumbered state Nursing License.
• Experience in Assisted Living or Memory Care is a bonus.
• 1+ years of experience as an LPN.

Responsibilities

• Deliver nursing care to residents in a safe, at-home environment.
• Administer medications and treatments per care plans.
• Collaborate with team to provide quality care reflecting core values.
• Maintain accurate, up-to-date documentation.
• Provide excellent service to residents, families and medical partners.
