Job summary

Sapphire Nursing at Wappingers in Dutchess County, NY seeks LPNs for immediate openings on multiple shifts (11pm, 7am, 3pm–11pm, 7am–3pm). Under supervision, LPNs provide direct nursing care and support residents’ comfort and well-being.

Applicants should have NYS LPN licensure and completion of an approved nursing program. LTC experience is a plus; offers competitive pay, full benefits, and growth opportunities in a premier facility.

Qualifications

• Must have completed an educational program and NYS licensure as a LPN.
• Prior experience in a long term skilled nursing facility is a plus.

Responsibilities

• Delivers nursing care in structured and routine situations.
• Administers and records medications and treatments as assigned.
• Prepares for and assists Nurses with diagnostic procedures and treatments.
• Reinforces and provides health care instructions to patients and families.
• Participates with team members in planning, implementing and evaluating nursing care and patient activities.
• Assists in admission, transfer and discharge of patients.
• Observes and reports patients’ vital signs and general condition.
• Maintains patient charts with observations and responses to care.
• Maintains safe and comfortable environmental conditions.
• Assists with unit management and may assume unit charge as assigned.
• Directs other technicians and nurse’s aides in patient care.

Sapphire Nursing at Wappingers, a premier long term skilled nursing facility located in Dutchess County, New York has immediate openings(both full and part-time) for LPN’s for the 11pm and 7am, 3pm to 11pm shifts and 7am to 3pn shift). Under supervised professional and skilled guidance, our LPN’s give direct nursing care to assigned residents and perform other services necessary for their personal and comforts needs. Responsibilities

• Delivers nursing care in structured & routine situations.
• Administers and records medications and treatments as assigned.
• Prepares for and assists Nurses and others with diagnostic procedures and treatments.
• Reinforces, supports and provides health care instructions to patients and their families.
• Participates with other team members in planning, implementing and evaluating nursing care and other patient activities.
• Assists in the admission, transfer and discharge of patients.
• Observes and reports patients’ vital signs, symptoms and general condition to the appropriate individuals(s).
• Records on patient charts observation of patient’s responses to nursing care rendered.
• Maintains environmental conditions conducive to the comfort and safety of patients.
• Assists with unit management and/or assumes charge of unit as assigned.
• Performs other nursing functions related to patient care and nursing service, as delegated by the Nursing Service Administration.
• Guides and directs other technicians and nurse’s aides in rendering patient care.
• Maintains clinical competence through participation and attendance at continuing educationprograms assistance with activities of daily living.

Requirements
