Delta Health Center Inc. seeks a licensed practical nurse to support patient triage, referrals, and coordination in a busy primary care setting.

This full-time, on-site role requires IV certification and completion of an LPN/LVN credential, with emphasis on organizing referrals and maintaining accurate documentation. You will triage patients, expedite referrals to specialty offices, answer calls, remind patients of appointments, and work independently to ensure timely care.
