Job summary

Phoenix Home Care & Hospice is seeking LPNs & RNs to deliver compassionate, patient-centered home nursing for children and young adults. You will provide essential one-on-one care in patients’ homes, collaborating with families and care teams to tailor plans for complex medical needs.

We offer full-time, part-time, and PRN shifts with specialized, hands-on training to build expertise and confidence. You’ll be valued for your dedication and impact on families’ lives as they thrive at home.

Qualifications

• Current and active LPN or RN license.
• Pediatric experience preferred, but training provided.
• Passion for patient-centered care.
• Hands-on training available at Phoenix.

Responsibilities

• Provide one-on-one home-based nursing care to pediatric and young adult patients.
• Build trusted relationships with patients and their families.
• Implement and monitor individualized care plans in the home setting.
