Job summary

Phoenix Home Care & Hospice is seeking LPNs and RNs to deliver compassionate, pediatric home health care in patients' homes across Massachusetts. You’ll provide meaningful, one-on-one nursing care, focusing on the unique needs of children and young adults.

• Current and active LPN or RN license in MA.
• Experience in pediatric/home health preferred but training is provided.
• Strong communication and family-centered care.

Responsibilities

• Provide one-on-one pediatric/home-health nursing in patient homes.
• Monitor vitals, administer medications, and manage care plans.
• Collaborate with families and care teams to support daily living and health goals.
