Job summary

Lakeview Manor Nursing and Rehabilitation is seeking a full-time LPN/RN licensed nurse to work night shifts. The role includes medication administration, supervision of Nursing Assistants, and coordination with physicians to ensure quality resident care.

The RN/ LPN will lead rounds, assess residents’ needs, and communicate with incoming and outgoing staff to maintain continuity of care, while adhering to facility policies and procedures.

Qualifications

Responsibilities

Lakeview Manor Nursing and Rehabilitation is seeking a full time LPN/RN to work in our facility. Must be licensed.

LICENSED STAFF

RegisteredNurse/Licensed Practical or Vocational Nurse

JOBSUMMARY

He/Sheis responsible for the care of the residents in a designated area of thefacility including the administration of medications as ordered by theresidents’ physician. He/Shecommunicates with off going and arriving shifts of personnel to assure the continuum of quality of care.Makes rounds, evaluating the physical, emotional and social needs of the residentsmaking sure these are being met through the staff assigned to work with each groupof residents. Assesses assigned staff'sability to providefor each resident's needs and provides intervention as indicated. Supervises the care provided by the NursingAssistants assigned to the unit, providing intervention, and education as indicated. Makesadjustments in assignments as indicated. Prepares and administers medicationsand treatments according to procedures. Consults with physicians, receives, records and transcribesorders as required. Makes rounds with physicians whennecessary. Reviews resident's care plans and assures these plans are followed. Documents onrecords as per policy and procedures of the facility. Participates in the evaluation of theemployees working underhis/her supervision. Participates in staff meetings and in-services asrequired.

TheRegistered Nurse, Licensed Practical Nurse or Licensed Vocational Nurse is responsible to the Director of Nursingand/or the Assistant Director of Nursing.

EDUCATION

He/Shemust be a graduate of an accredited school of nursing currently registered withthe state agency for nursing licensure and hold a valid nursing license in the statehe/she is employed.

• 13. Mustdemonstrate ability to evaluate nursing staff needs and provide staff education timely to pr ovide quality care.
• 14. Mustdemonstrate ability to function as a cooperative team member with each memberof the Nursing Department and other departments.
• 15. Mustdemonstrate the ability to supervise assigned staff to assure the facilitiespolicies and procedures are followed and upheld.
• 16. Must demonstrate the ability to manage andsupervise the assigned unit in a productive,timely manner.
• 17. Must have a pre-employmentand an annual Tuberculosis screening examination.

RESPONSIBILITIES

• 1. Receives report from the offgoing shift.
• 2. Reviewscommunication systems to be knowledgeable of any unusual occurrences during the previousshifts.
• 3. Assigns Nursing Assistants andother designated personnel assuring each resident's needs will be met during that shift.
• 4. Givesreport to Nursing Assistants.
• 5. Makes rounds of the assigned unitfrequently during the shift providing intervention as needed. During the entire shift, will beattune to the needs of the unit while doing other nursing tasks.
• 6. Prepares and administersmedications as ordered. Reorders medications as needed in preparation forfuture shifts. Assesses and documents medication issues as per policy.
• 7. Provides special procedures as ordered.
• 8. Supervises the meal service foreach resident on the unit. Works with the staff to assure each resident is provided adequatenutrition for each meal.
• 9. Continually assesses and reassesses residents forrisk of complications as a result ofbeing in the facility. (i.e., skin breakdown, incontinence, contractures, reduced mobility, reducedinvolvement in bathing, dressing, or eating,weight loss, behaviors due to change in lifestyle, etc.)
• 10. Continuallyobserves progress and quality of work of the Nursing Assistants to assure that residents are turned,repositioned, toileted, walked, exercised, provided food and fluids, well groomed, involved in theirspecific restorative program,provides intervention as indicated, etc. Also observes the resident's environment to assure a clean andsafe environment is provided for each resident.
• 11. Document on nurse's notes as per policy. Assuresthere is accurate and timely documentationof any unusual observation, incident, or happening.
• 12. Admits residents as required.
• 13. Assists in the ordering and reviewing of results oflaboratory tests as indicated. Notifies physician of results.
• 14. Notifiesthe physician of unusual findings as indicated.
• 15. Assistswith auditing charts.
• 16. Assistswith answering and correcting consultant reports.
• 17. Reviews, signs and provides intervention asindicated for Nursing Assistants documentation.
• 18. Counts required medications with on coming and off going shifts.
• 19. Makesappropriate notes in the 24 hour report to assure adequate communication is maintained fornursing.
• 20. Assistsin keeping other departments informed of resident's change of condition as indicated.
• 21. Assistsin providing in-services for assigned staff.
• 22. Keeps nurse's station andmedication room clean. Assigns Nursing Assistants to keep each ancillary room clean and orderly. Assigns staff to keepequipment clean and proper conditionfor use.
• 23. Reviewsand updates Medication Administration Record, Treatment Administration Record, and Nursing Care Flow Recordsfor the upcoming month.
• 24. Makesrounds toward end of shift to assure area is in order for the oncoming shift.
• 25. Givesreport to oncoming shift.
• 26. Reviewsand assures there is adequate staffing for the oncoming shift. Replaces known absences in atimely manner.
• 27. Communicates all changes in condition,incidents, new orders, to resident and/or resident representative.
• 28. Performs other tasks as assigned.
• 30. Attendsrequired staff in-services, meetings, orientation, and continuing education activitiesas required.
• 31. Is aware ofand adheres to Patient’s Bill of Rights and confidentiality of patient informationincluding HIPAA regulations.
• 32. Interacts withpatients and family members, co-workers, clinical and ancillary staff in anon-judgmental, supportive and calm manner.
• 33. Is aware ofPatient Abuse Reporting Law.
• 34. Participatesas assigned in the orientation of new staff and demonstrates a positive,helpful and enthusiastic attitude.
