Job summary

Salem Hills Rehabilitation & Healthcare, a leading long-term care facility in Westchester County, is seeking an LPN for full-time or part-time roles with every other weekend requirements. This beautiful campus hosts both Waterview Hills and Salem Hills, committed to high-quality subacute and long-term care.

Responsibilities include delivering direct resident care, administering medications, monitoring vitals, and collaborating with the care team to update Nursing Care Plans.

Qualifications

• Graduate of an approved school of Nursing.
• Licensed & currently registered in New York as an LPN.
• Experience in long-term care setting preferred.

Responsibilities

• Provide daily support to the unit leadership and respond to resident/family needs.
• Deliver direct resident care using the nursing process and set priorities for safety and quality care.
• Administer standard nursing treatments and medications and monitor resident responses.
• Communicate with care team to revise Nursing Care Plans and review lab results and concerns.
• Monitor vital signs and document treatments and resident responses.
• Oversee CNA assignments and activities to ensure appropriate care delivery.
