Job summary

As an LPN, you will provide direct patient care under supervision, administer medications, support daily living activities, and collaborate with an interdisciplinary team to develop care plans.

Qualifications

• Current LPN licensure in the state of Pennsylvania.
• Minimum 1 year of experience in long-term care or rehabilitation setting.
• Ability to work effectively in a fast-paced environment.
• Strong communication and interpersonal skills.

Responsibilities

• Provide direct patient care under the supervision of registered nurses and physicians.
• Assist with daily living activities such as bathing, dressing, and grooming.
• Administer medications and treatments as prescribed by physicians.
• Develop and implement individualized care plans in collaboration with interdisciplinary team members.
• Monitor and report changes in patient condition to healthcare team members.
