Job summary

Pinnacle Group of Hudson Valley Ltd is seeking an LPN to join our on-site care team in Maine. The role includes collaborating with a team to develop and review personalized care plans, evaluating residents, and administering medications in accordance with protocols.

The successful candidate will document medical records accurately, communicate with residents and families, and advocate for residents' rights. Other duties as needed or assigned.

Qualifications

• Current Maine LPN license is required.
• Experience in a nursing home, assisted living, or hospital is preferred.

Responsibilities

• Collaborate with a team to develop, implement, and review personalized care plans for residents
• Evaluate patients to collect data, observe conditions, and report changes in condition.
• Administer medications as prescribed, ensuring accuracy and adherence to protocols
• Accurately document and maintain accurate records of medical information, care plans, and incidents to ensure regulatory compliance and consistency of care.
• Communicate effectively with residents and residents' families
• Advocate for residents' rights and preferences, ensuring their voices are heard and respected in all aspects of care.
• Other duties as needed or assigned
