Job summary
Northridge Health and Rehabilitation Center is seeking Licensed Practical Nurses (LPNs) to join our caring team at 1445 North 7th Street, Manitowoc, WI. We offer full-time, part-time, and PRN options across AM, PM, and NOC shifts, with opportunities for advancement including ADON and Unit Manager roles for qualified staff.

Qualifications
- Current, active LPN license in Wisconsin.
- Strong communication and teamwork skills.
- Compassion for and commitment to caring for the aging population.
- Ability to work collaboratively in a fast-paced healthcare environment.
- New graduate LPNs are strongly encouraged to apply.
Responsibilities
- Administer medications and monitor residents' vital signs.
- Provide compassionate, person-centered nursing care.
- Implement and support individualized resident care plans.
- Complete accurate clinical documentation.
- Lead and support CNA staff.
- Promote resident safety, comfort, and dignity.
- Communicate and collaborate with Nurse Managers and the interdisciplinary care team.
- Monitor residents' conditions and communicate changes appropriately.
