Job summary

Optalis Healthcare in Columbus, OH is seeking a Licensed Practical Nurse to coordinate resident care and ensure compliance with local, state, and federal regulations in a long-term care setting.

You will partner with the Administrator, Director of Nursing and inter-disciplinary teams to implement care plans, monitor resident needs, answer call lights, document accurately and support survey readiness.

Qualifications

• LPN license in good standing is required.
• High School diploma or GED required.
• CPR certification required.
• 2 years long-term care experience preferred.

Responsibilities

• Coordinates resident care with Administrator, Director of Nursing and other disciplines to ensure quality care.
• Ensures care plans are implemented and updated based on resident needs.
• Responds to call lights and communicates needs to appropriate staff promptly when not able to meet requests.
• Completes daily rounds and provides shift reports to oncoming personnel with relevant resident information.
• Supports survey readiness and quality assurance activities.
• Demonstrates strong communication and documentation abilities; maintains organization and time management.
