Responsibilities

• Deliver patient-centered care as part of a collaborative nursing team.
• Monitor residents, track changes in condition, and document accurately.

• Collaborate with RNs, physicians, and therapy teams to support care plans.
• Educate residents and families.
• Maintain clear, compliant documentation in EMR.
• Assist with supervision and support of CNAs.
• Help maintain a safe, clean, and supportive environment.
