Available Shifts: PRN – Day (6a-6p) and Night (6p-6a) availability

Job Duties

The LPN coordinates with the health care team to deliver quality care and restorative services that maximize resident health potential, functional capacity, and quality of life. This includes providing direct and indirect nursing care, resident assessment, care planning, intervention and evaluation to meet resident needs.

Responsible for the direction and supervision of nursing assistants, ensuring the provision of quality care and health services in compliance with professional nursing standards and state and federal regulations.

Qualifications

• Successful completion and graduation from an accredited school of Licensed Practical Nursing
• Current, unencumbered license to practice Licensed Practical Nursing in Louisiana
• Current CPR certification
• At least one year of experience
• Long‑term care experience and experience with residents in advanced stage dementia – a plus
• Ability to maintain confidentiality, relate well with seniors and their families, and understand written and verbal instructions
• Good attendance and punctuality

Essential Functions

Demonstrates Leadership

• Delegates tasks appropriately
• Supervises and coordinates care/service delivery by nursing assistants, restorative nursing assistants, and ancillary services
• Serves as charge nurse in the absence of an RN
• Communicates resident information effectively to other multidisciplinary team members
• Ensures resident safety, security, and rights at all times

Coordinates Resident Care and Services

• Coordinates admissions, transfers, and discharges
• Initiates assessment, care plan, intervention/treatment, and outcome measurement
• Consults with attending physicians to provide quality resident care
• Obtains vital signs and performs blood sugar tests as ordered and reports abnormalities to the physician
• Requisitions diagnostic and therapeutic services as ordered by the physician and in accordance with established procedures
• Reviews resident charts for accuracy and completion
• Collaborates with the interdisciplinary team to promote quality resident care and services
• Delivers professional nursing services such as assessment, treatment & outcome evaluation, medication administration, parenteral and enteral therapy, airway maintenance, wound care, and care of the dying resident
• Includes family and significant others in resident assessment, care planning, and treatment when appropriate
• Handles cleaning and storage of equipment

Administers Medications

• Obtains, prepares and administers medications, including oral, topical, and injectable, as ordered and in accordance with policy and procedure
• Reviews medication records for completeness and accuracy
• Ensures narcotic records are accurate and reports discrepancies to the Director of Nursing
• Disposes of drugs as required according to policy and procedure
• Assists with resident dining tasks such as escorting residents to the dining room and aiding in meal serving and feeding

Completes Accurate Nursing Documentation

• Files forms/charts from admission through discharge in accordance with state and federal regulations
• Accurately documents verbal/telephone physician orders and nurse notes
• Records all resident accidents/incidents per policy and procedure
• Communicates changes or new orders to appropriate interdisciplinary team members
• Reports discrepancies noted in physician orders, charting, or medication/treatment administration to the Director of Nursing
• Performs routine documentation duties including behavioral monitoring forms and incident reports

Physical Requirements

Ability to stand and walk for extended periods; ability to lift and assist with resident transfers.
