The Wellstar Health System is seeking a dedicated LPN for Home Health PRN visits across communities south of Douglasville, Kennestone, and Cobb/Cherokee counties. You will travel to patient sites, providing direct care for high-acuity cases recovering after hospital discharge, under RN supervision.
Ideal candidates have at least one year of med-surg or relevant experience and a commitment to compassionate, patient-centered care within Georgia's integrated health system.

This highly committed LPN will work PRN ( minimum availability 2 shifts a month but please be flexible and committed per community needs in real time) traveling to patient site for home health patients mainly post Wellstar inpatient or hospital care- varying degrees of acuity so must have exposure to patients discharged from hospitals with higher acuity issues as they recover
since PRN must have relevant experience in higher acuity home care with patients leaving the hospitals- ideally at least one year of med surg or relevant
Territory adjacent to Wellstar sites mainly South of Douglasville over beyond Kennestone, Woodstock, Cobb/Cherokee County, etc )

The LPN works under the direction of a Registered Nurse or Manager and functions within the LPN Scope of Practice. The LPN is responsible for providing direct patient care to patients with medical/surgical needs. This includes age appropriate interventions that meet physical, emotional, developmental, psychosocial and educational needs. The LPN is responsible for coordination and delivery of high quality care. LPN participates in call rotation and weekend visits as needed. The LPN can serve as a preceptor for new LPN employees. They are responsible for attending and participating in staff meetings, in-services, special projects and continued education.
All certifications are required upon hire unless otherwise stated.
