Job summary

Team Select Home Care is seeking a licensed practical nurse to provide one-on-one in-home care for pediatric and/or adult patients with complex medical needs in Fort Wayne, IN. You will work under an RN, administer meds, educate families, and document visits using EMR.

We offer flexible scheduling (days/nights, weekdays/weekends), weekly pay, and a mentorship program for new grads, with a supportive leadership team and opportunities for ongoing training.

Qualifications

• Active LPN license.
• CPR certification.
• Excellent communication and critical thinking.
• Passion for home care with pediatric/adult patients.

Responsibilities

• Provide skilled nursing care under the direction of an RN.
• Administer medications and treatments per individualized plans.
• Educate and support families in-home care techniques.
• Observe and report changes in patient condition.
• Document visits using EMR system.
• Be a trusted part of a long-term care team focused on safety, comfort and continuity.
