Job summary

Sarah Bush Lincoln is seeking an LPN to support physician practices in Fayette County. You will assist with patient exams, coordinate follow-up tests, document care in the medical record, and teach patients about their plan of care.

You may perform lab duties, process prescriptions, and ensure pre-certifications are obtained. Applicants should have a high school diploma, be CPR/BLS certified within 90 days, and be a graduate of an accredited LPN program; Illinois LPN license is required.

Qualifications

Responsibilities

• Assists physician staff in clinical visits.
• Answers phone calls from patients, hospitals, and offices and directs information to the appropriate provider.
• Coordinates follow-up care including scheduling tests and referrals.
• Documents assessment, care provided, and data collection in the medical record.
• Performs laboratory duties including phlebotomy and waived testing.
• Submits prescriptions and refills to the pharmacy in a timely manner.
• Teaches patients and families about the plan of care and documents education.
