SMJMG Forest Lakes in Charlottesville, VA is hiring an LPN on site to join a physician practice. New graduates are welcome to apply, with a sign-on bonus up to $8,000 available.

Schedule is Mon-Fri, 8a-5p, full-time, 40 hours, with no nights or holidays. The LPN supports providers in ambulatory care, rooms patients, takes vital signs, administers medications and vaccines, collects specimens, and documents in the EHR.

Qualifications

• Completion of an accredited practical nursing program leading to LPN/LVN eligibility.
• A high school diploma or equivalent is required.
• Experience in ambulatory or physician practice settings is preferred.

Responsibilities

JOB DESCRIPTION

The Licensed Practical Nurse (LPN) in a physician practice or clinic supports providers and registered nurses in delivering direct patient care in the ambulatory setting. The role rooms patients, gathers and records health information, measures vital signs, administers medications and immunizations, collects blood specimens, and performs simple diagnostic tests. The LPN prepares patients and rooms for examinations, assists with procedures, and provides patient education and follow-up instructions within scope. Working closely with the care team, the LPN helps manage patient flow, responds to messages and refill requests under provider direction, and may direct the work of medical assistants and aides. Accurate documentation in the electronic health record supports continuity, care coordination, and safe outpatient care.

MINIMUM REQUIRED EDUCATION

Completion of an accredited practical nursing program leading to LPN/LVN eligibility is required. A high school diploma or equivalent is required. Experience in an ambulatory or physician practice setting is preferred.

MINIMUM REQUIRED EXPERIENCE

One to two years of nursing experience is preferred, ideally in an ambulatory or outpatient setting. New graduates of an accredited practical nursing program may be considered.

MINIMUM REQUIRED CERTIFICATION / LICENSURE

Current, unencumbered state Licensed Practical Nurse (LPN/LVN) license is required. Successful completion of the NCLEX-PN examination is required. Basic Life Support (BLS) certification is required.

MINIMUM REQUIRED COMPETENCIES

Skilled in ambulatory nursing care within the LPN scope. Accurate vital sign measurement, medication and immunization administration, and specimen collection. Ability to room patients, prepare rooms, and assist with office procedures. Strong patient education and clear communication. Efficient support of patient flow in a busy clinic. Comfort documenting in the electronic health record and using clinical messaging tools. Ability to guide medical assistants and aides. Dependable follow-through within scope.

RESPONSIBILITIES

• Follows infection prevention and safety standards consistent with DNV and CMS requirements.

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $26.10 - $38.70 hourly. Additional compensation may be available for this role, such as shift differentials, on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities.
