Job summary

Life Care Centers of America is seeking an LPN for the Personal Care Unit. The role focuses on delivering quality nursing care in a post-acute setting, under Life Care standards, and involves supervising CNAs and charting patient information.

Ideal candidates will be licensed LPNs, have completed an accredited program, and hold CPR certification; familiarity with geriatric care is preferred. This is a full-time position with day or night shifts in Nashville, TN.

Qualifications

• Attended an accredited LPN program.
• Currently licensed/registered as an LPN/LVN in good standing.
• CPR certification required upon hire or during orientation.

Responsibilities

• Deliver quality nursing care to patients on the personal care unit.
• Assign duties and provide clinical supervision to CNAs.
• Chart appropriately and timely.
• Monitor and report changes in patient condition and maintain a positive patient attitude.
• Assist in patient evacuation and work with an interdisciplinary team.

Position Summary

The LPN Personal Care Unit Licensed Practical Nurse delivers quality nursing care to patients through interpersonal contact and provides care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well being in accordance with all applicable laws, regulations, and Life Care standards.

Education, Experience, and Licensure Requirements

• Attended an accredited LPN program
• Currently licensed/registered in applicable State. Must maintain an active Licensed Practical/Vocational Nurse (LPN/LVN) license in good standing throughout employment.
• One (1) year geriatric nursing experience preferred
• CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.

• Make independent decisions when circumstances warrant such action
• Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
• Implement and interpret the programs, goals, objectives, policies, and procedures of the department
• Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation
• Maintains professional working relationships with all associates, vendors, etc.
• Maintains confidentiality of all proprietary and/or confidential information
• Understand and follow company policies including harassment and compliance procedures
• Displays integrity and professionalism by adhering to Life Care’sCode of Conductand completes mandatoryCode of Conductand other appropriate compliance training
