Job summary

Brookhaven Center for Rehabilitation & Healthcare is seeking a Licensed Practical Nurse for per diem and weekend shifts in New Jersey. The LPN will provide hands-on nursing care to residents, administer medications, and support the clinical team to ensure compassionate, high-quality care.

Immediate openings available. The position requires an NJ LPN license and graduation from an accredited LPN program, with ability to work independently and with a care team.

Qualifications

• Current NJ LPN license is required.
• Graduate of an accredited LPN program.
• Ability to work independently and as part of a care team.
• TB screening upon hire is required.
• Strong communication and organizational skills.

Responsibilities

• Provide direct nursing care per individualized care plans.
• Administer medications and treatments with accurate documentation.
• Perform clinical assessments and monitor residents’ conditions.
• Assist physicians during rounds and exams.
• Communicate with residents’ families and care team.
• Ensure safety, health, and quality regulations are followed.
