Qualifications

• Active state LPN license.
• Compassionate, patient-centered care.
• Experience with elderly and chronic conditions preferred.

Responsibilities

• Administer medications as prescribed by healthcare providers.
• Monitor vital signs and record medical information.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Provide wound care and other nursing interventions as needed.
• Supervise and support CNAs in delivering care.
• Ensure compliance with regulatory standards and protocols.
