Lantern Hill by Erickson Senior Living in New Providence, NJ, seeks a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for per diem shifts. You will build relationships with residents and families, assess clinical needs, update care plans, supervise care staff, and administer medications.

Experience in assisted living, home health, or long-term care preferred; state license required; CPR certification; EMR experience a plus.

Qualifications

• 1+ year of experience in assisted living, home health, or long-term care preferred.
• Licensed by State as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
• CPR certification required.
• Experience with electronic medical records (EMR) preferred.
• State regulations may apply and take precedence over the job description.

Responsabilités

• Develop relationships with residents and families, focusing on resident-first care.
• Assess, observe, and document residents' clinical condition; notify RN/provider when needed.
• Update or modify care plans as resident needs change.
• Supervise and lead care associates.
• Administer medications timely and accurately per orders.
• Participate in meetings related to resident outcomes and care.

Compensation

Between $31.00 - $42.00 per hour. Commensurate with experience.

How you will make an impact

• Developing relationships with the residents and their families using a consistent caregiver approach supporting the resident’s physical, spiritual, emotional, and clinical needs and focusing on placing the resident first.
• Assessing, observing, and evaluating resident’s clinical condition, document findings in the resident electronic medical record, and notify the provider and/or RN when necessary.
• Updating or modifying care plans as needed to reflect any changes to a resident’s care and service needs.
• Supervising and leading care associates
• Administering medications timely, accurately, and according to orders.
• Participating in meetings related to resident outcomes and care
• Responding to residents, family, or responsible parties in a timely and professional manner
• Participating in the review of clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc.
• Other duties as assigned by RN clinical leader

What you will need

• Minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
• Licensed by State as a Licensed Practical Nurse.
• CPR certification required.
• A passion to care for seniors and be a strong resident advocate.
• Experience with electronic medical records is preferred.
