Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of practice.
• Ability to work rotating shifts including days/nights/weekends as required.

Responsibilities

• Collects data and conducts focused nursing assessments.
• Participates in development and modification of the patient care plan.
• Implements nursing interventions within the plan of care.
• Evaluates responses to interventions.
• Maintains safe and effective nursing care.
• Advocates for patient interests.
• Communicates and collaborates with patients and health team members.
• Provides health counseling and teaching.
• Delegates and assigns nursing interventions to implement the plan of care.
