SGA H. Care Staffing LLC is seeking Licensed Practical Nurses (LPN) to provide direct patient care for facilities in Clifton Park, NY. Nurses will support RNs and CNAs, monitor vitals, assist with daily living activities, and ensure accurate documentation of patient records.

Qualifications

• LPN license in New York is required.
• Direct patient care and collaboration with RN/CNA as part of the care team.

Responsibilities

• Monitor patient’s vitals and record them in their file.
• Maintain cleanliness and comfort, including bathing, feeding, and dressing.
• Assist with basic medical tasks, such as administering medication.
• Update patient records confidentially and report changes to the medical team.
• Explain treatments and procedures to patients and families.
• Move patients with equipment as needed.

A Licensed Practical Nurse (LPN) provides direct care to patients and is a critical part of a healthcare team. An LPN usually assists a Registered Nurse (RN) and often oversees one or more Certified Nursing Assistant(s) (CNA) to carry out care plans and maintain patient comfort and well-being. LPNs take on a direct role in providing personal care to those in their charge, often assisting with all areas of the patient’s daily living needs. This enables LPNs to have a hands‑on understanding of the care needed and offers the ability to make a positive impact on health outcomes.

• Monitor patient’s vitals and record them in their file.
• Maintain the cleanliness and comfort of each patient, including bathing, feeding, and dressing.
• Assist nursing and medical staff with basic medical tasks, such as administering medication.
• Update patient records in a confidential manner.
• Evaluate the patient’s well-being, reporting any needs to medical staff.
• Explain treatments and procedures to patients and families.
• Change dressings and bandages, ensuring patients are healing properly.
• Move patients from wheelchairs, beds, and gurneys.
