Job summary

BMC Berkeley Medical Center is seeking an LPN to provide direct patient care, following the patient plan and collaborating with RNs and therapists to promote comfort and recovery.

Responsibilities include medication administration, patient data collection, and assisting in exams and procedures. The role requires BLS, an active LPN license, and flexibility for three 12-hour night shifts per week, with every other weekend and rotating holidays.

Qualifications

• Licensed LPN in the state of practice or multi-state license (eNLC preferred).
• BLS certification required within 30 days of hire.
• Background check (state and federal if applicable) required.

Responsibilities

• Provide direct patient care per care plan and in guidance of supervising nurse or therapist.
• Perform designated therapeutic procedures and assist with exams as assigned.
• Observe and document patient status and report changes promptly.
• Assist physicians and staff with examinations and procedures.
• Contribute to nursing assessment by collecting and recording data.
• Monitor patient condition and communicate deviations to the team.
• Support development of the care plan and patient education.
• Document nursing interventions and responses in the chart, timely.
• Administer medications via IM, subcutaneous, IV, oral, nebulation or MDI as directed.
• Seek supervision as needed when implementing nursing techniques.

MINIMUM QUALIFICATIONS

EDUCATION, CERTIFICATION, AND/OR LICENSURE

1. 1. Current Licensed Practical Nurse license issued by the state in which services will be provided or current multi-state Licensed Practical Nurse license through the enhanced Nurse Licensure Compact (eNLC).
2. 2. Obtain certification in Basic Life Support within 30 days of hire date.
3. 3. State criminal background check and Federal (if applicable), as required for regulated areas.

8. 8. Assists in assigned patient and family education and rehabilitation by reviewing patient needs with the overseeing role and following the teaching plan.
9. 9. Documents nursing interventions and responses to care, communicating to appropriate members of the health care team.
10. 10. Accurately and safely administers intramuscular, subcutaneous, IV, oral, nebulization and/or MDI delivered medications as assigned.
11. 11. Obtains instruction and supervision as necessary from the overseeing role when implementing nursing technique or practices.
