Job summary

AdvisaCare is seeking Registered Nurses to provide skilled, private duty and hospice care in Lansing, MI and surrounding areas. You will help transition patients from hospital to home, building close relationships and delivering personalized care plans.

We offer per-diem, part-time and full-time options with flexible scheduling, competitive pay, and a supportive team environment focused on patient quality of life.

Qualifications

• Current MI Nursing License required.
• Proven nursing experience in home health, private duty, or hospice care preferred.
• CPR certification and ability to pass background check/drug screen required.

Responsibilities

• Manage records, complete necessary paperwork, and perform patient assessments.
• Provide individualized care plans and support in patient homes.
• Collaborate with interdisciplinary teams to ensure safe transitions from facilities to home.

Responsibilities

• Manage records, complete necessary paperwork, and perform patient assessments
• Ability to work independently
• Participate in training and transitioning of the patient from a facility to the home setting.
• Comply with all policies and standards of safety and infection control.
• Perform all duties and responsibilities in a courteous and customer-focused manner in accordance with AdvisaCares standards of practice.

• Current MI Nursing License
• Proven Nursing Experience
• Familiarity with Professional and Technical Emerging Knowledge
• Problem Solving Skills and Ability to Multi-Task
• Reliable Transportation
