Job summary

Rochester Regional Health is seeking a Licensed Practical Nurse (LPN) for Park Ridge Living Center in Rochester, NY. The part-time night shift (11:00pm–7:00am) with every other weekend requirement provides care to long-term and post-acute residents.

You will lead CNAs, ensure accurate documentation, and participate in ongoing care initiatives while supporting a CMS 5-Star facility rated among the best nursing homes.

Qualifications

• Graduate from an accredited practical nursing program.
• Current New York LPN license.
• Basic Life Support (BLS) certification.

Responsibilities

• Timely, accurate medication administration and required documentation.
• Perform treatments and document progress per physician orders.
• Ensure complete resident care documentation in the EMR.
• Initiate appropriate care pathway tools as directed.
• Receive/give reports to staff per departmental protocols.
• Monitor residents and report condition changes to charge nurse/physician.
• Complete MAR pre-reviews and related observations.
• Support CNA team with direct ADL care.

Education

Graduate from an Accredited School of Practical Nursing

Current NYS LPN license

BLS Certification (AHA)
