Job summary

Woodstock Health Care Center in Ohio is hiring a Licensed Practical Nurse (LPN) for full-time night shifts (6pm-6am) and PRN. The role provides direct nursing care under RN supervision, administers medications, and supports care plan implementation to ensure safe, high-quality resident care.

Responsibilities include assessments, admissions duties, discharges, wound care, leading CNAs, and responding to emergencies.

Qualifications

• LPN license valid in Ohio.
• Ability to administer medications and follow care plans.
• Experience in long-term care setting preferred (not required)

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
