Job summary

Pikeville Nursing and Rehab in Pikeville, KY is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents, administer medications, and support care plans.

You will work with RNs, CNAs and physicians in a skilled nursing facility, ensure accurate documentation, monitor vitals, and respond to changes in residents' conditions.

Qualify with Kentucky LPN licensure, graduation from an accredited program, CPR/BLS, and good communication.

Qualifications

  • Licensure: Valid LPN/LVN license in Kentucky
  • Education: Graduation from an accredited LPN program
  • Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred
  • Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations.
  • Communications: Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.
  • Physical Requirements: Ability to lift and move patients, perform physical assessments, and stand for extended periods.

Responsibilities

  • Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
  • Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
  • Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.
  • Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
  • Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
  • Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
  • Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.

Job Summary

Work Environment
