Job summary

Canyon View Care Center in Colorado is seeking a licensed practical nurse (LPN) to provide direct nursing care to residents and supervise day-to-day nursing activities in a long-term care setting.

The LPN will follow physician orders, collaborate with RNs and physicians, and ensure adherence to federal, state, and local standards of care. A $3,000 sign-on bonus is available and new higher wages are offered under the comp plan.

Qualifications

• Active LPN license in Colorado.
• Current CPR certification.
• Graduate of an accredited school of Nursing.
• One year experience in a long-term care environment preferred.
• Valid CO license to practice as LPN.

Responsibilities

• Assess patients through physical exams and pertinent tests to determine health status.
• Administer medications and treatments as ordered.
• Participate in care planning and care coordination.
• Supervise LPNs and certified nursing assistants.
• Communicate changes in resident conditions with physicians and RNs.
• Document assessments and care in line with standards and company policy.

POSITION SUMMARY

A licensed practical nurse (LPN) provides care and treatment in accordance with physician orders. Works in a team environment and within scope of practice as defined by State. The LPN works under the direction of physicians and registered nurses (RNs). The primary responsibilities of your role in your assigned location is to provide direct nursing care to residents and to supervise the day-to-day nursing activities following the current federal, state and local standards, guidelines and regulations that govern long term care and skilled nursing facilities. You are entrusted to ensure that the highest degree of quality care will always be provided to our residents.

DUTIES AND RESPONSIBILITIES

• Assesses patients by physical examination, including pertinent diagnostic testing to determine health status.
• Administers medications and treatments.
• Participates in the care planning process.
• Supervises LPNs and certified nursing assistants.
• Communicates with physicians regarding changes in resident's conditions, diagnostic test results, etc.
• Documents assessments and care in compliance with standards of care and company policy.
• Educates patients and their families on health-related issues.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.

KNOWLEDGE, SKILLS, ABILITIES & QUALIFICATIONS

• Knowledge of computer systems including Point Click Care
• Knowledge of nursing and medical practices, procedures and standards of care
• Graduate of an accredited school of Nursing.
• Valid State licensure as an LPN
• Current CPR certification.

Experience: One-year experience in a long-term care environment (preferred)

License: Current, unencumbered, active license to practice as an LPN in Colorado, CPR Certification
