Heritage Hall in Virginia is seeking an LPN to join our Memory Care Unit, delivering direct, compassionate care to residents and supporting their families.

You will work as part of a dedicated team, using dementia-care best practices, with opportunities for professional growth and a strong benefits package.

Qualifications

• Current LPN/LVN or RN license in the state of employment.
• Current CPR/BLS certification as required.
• Previous long-term care, assisted living, or memory care experience preferred.
• Strong assessment, communication, organization, and leadership skills.
• Knowledge of dementia care principles and person-centered care preferred.

Responsibilities

• Provide direct care to memory care residents and monitor needs.
• Support care planning and documentation for residents.
• Collaborate with the care team to implement dementia care plans.
• Communicate with residents, families, and team members to ensure high-quality care.
