Job summary

Danbury Senior Living is seeking an experienced LPN to lead and coordinate dementia-focused programming at our community in Ohio. You will assess resident needs, plan activities, and collaborate with nursing and life enrichment staff to deliver person-centered care.

The role requires organizing training, supporting staff, and ensuring compliance with documentation and regulatory standards. Evening, weekend, and holiday availability is expected.

Qualifications

• Must be an LPN.
• Experience with Alzheimer’s and other dementia individuals.
• Two years of programming experience including plans of care, scheduling staff, coordinating with POAs and families, planning activities and working with dementia residents.
• Background in nursing and providing one-on-one care for seniors.
• Flexible schedule, including evenings, weekends and holidays as needed.

Responsibilities

• Plan and coordinate a therapeutic program meeting resident needs across spiritual, social, emotional, physical, and intellectual domains.
• Assess resident characteristics and, with other departments, plan and organize program content.
• Monitor daily neighborhood functioning to ensure continuity and appropriate changes in the program.
• Assist with in-service training and education of care staff alongside Nursing and Life Enrichment leadership.
• Establish and maintain teamwork through effective communication and staff consultations.
