Viability Inc. is seeking an LPN to provide nursing care to individuals with disabilities in West Springfield, MA. The role focuses on medication administration, G-tube feeding, and health assessments, with documentation of all medications and adherence to service plans.

The LPN will ensure programmatic and contractual obligations are met, align with regulatory requirements, and may supervise nursing staff. Requires MA LPN license and a year of relevant experience.

Formación

• Must hold a MA LPN License.
• One year of human service experience required.
• Must have a valid driver’s license and a reliable vehicle.
• Demonstrated reliability and dependability.
• Knowledge of and respect for diverse populations.

Responsabilidades

• Administer medications and manage basic nursing duties per service plans.
• Perform G-tube feeding and associated care.
• Document all prescribed medications accurately for members.
• Assess health concerns and develop/implement therapeutic programs.
• Provide supervision and training to nursing staff in identified areas.

Conocimientos

LPN license

One year of human service experience
