Job summary

The GreenFields Continuing Care Community is hiring a Licensed Practical Nurse for 3:00pm - 11:00pm shifts, including every other weekend. You will administer nursing care, coordinate with CNAs, and help implement each resident's plan of care.

Qualifications

• Two years' experience in SNF, acute care, or long-term setting.
• Current LPN license in good standing.
• CPR/BLS certification.
• First Aid certification (if required).

Responsibilities

• Assess and provide nursing care according to the plan of care.
• Administer medications and treatments and document responses.
• Coordinate with CNAs and support staff.
• Update ISP or care plan and communicate with physicians.
• Ensure safe patient transfers and hospital readmissions when needed.
