Twilight Gardens in Norwalk, Ohio is seeking a compassionate Licensed Practical Nurse (LPN) to join our team. You will provide direct nursing care to residents and collaborate with the care team to ensure excellent resident outcomes.

Qualifications

• Active Ohio LPN license is required.
• Long-term care experience preferred, but not required.
• Strong communication and teamwork skills.

Responsibilities

• Administer medications and treatments as ordered
• Monitor resident conditions and report changes appropriately
• Complete charting and documentation accurately and timely
• Support and supervise nursing assistants
• Assist with admissions, discharges, and physician orders
• Communicate effectively with residents, families, and interdisciplinary teams
• Maintain compliance with facility policies and state regulations
• Help create a positive and compassionate environment for residents
