Job summary

Cynet Health is seeking an LPN/LVN to join our Long Term Care team for night shifts. Under the RN Supervisor, you will implement the plan of care, document patient data, and support nursing assistants with daily tasks.

The ideal candidate possesses an active LPN/LVN license, BLS, and is comfortable with medications, routine assessments, and responding to changes in resident conditions in a busy LTC setting.

Responsibilities

• Under RN Supervisor, carry out the medical plan of care.
• Oversee resident care to comply with nursing practices and regulations.
• Administer and record medications, treatments, and monitoring.
• Transcribe physician orders and complete monthly summaries.
• Review care plans with nursing assistants and update as needed.
• Notify RN Supervisor of significant changes in resident conditions.
• Document changes in resident condition and report to family and department heads.
• Assist physicians with rounds, examinations, and treatments.
• Collect laboratory specimens and arrange delivery.
• Supervise postmortem procedures on resident death.
• Encourage resident participation in activities and complete incident reports.

Description:

Under the direction of the RN Supervisor, the LPN Wing Supervisor carries out the medical plan of care.

Oversees the care provided to residents to ensure compliance with nursing practices and applicable regulations.

Administers and records medications, treatments, and monitoring as ordered.

Accurately transcribes physician orders and completes monthly summaries.

Reviews care plans with nursing assistants on summary days and updates as needed.

Notifies the RN Supervisor immediately of significant changes in resident conditions.

Reports and documents changes in resident condition.

Communicates resident transfers to family and department heads.

Completes transfer forms for all transfers.

Assists physicians with rounds, examinations, and treatments as needed.

Collects laboratory specimens and arranges for their delivery.

On resident death, supervises and assists with postmortem procedures.

Encourages resident participation in scheduled activities.

Completes incident reports accurately.

Documents weights and vital signs and follows up as necessary.

Cleans and stocks treatment trays daily and prepares daily reports.

Counts narcotics with another nurse at the beginning and end of shift.

Maintains an accurate inventory of resident belongings and documents any changes.

Maintains a clean and safe environment and reports potential hazards.

Supervises and directs resident care provided by nursing assistants.

Attends care plan conferences for assigned wing as needed.

Assists in the orientation of new nursing staff and adjusts staffing assignments as needed.

Conducts team conferences and ensures staff are familiar with changes in policies and procedures.

Monitors nurse aide documentation for compliance with regulations.

Provides direct care for residents as needed.

Documents drug disposal on resident charts.

Attends required in-services and meetings.

Assists nursing assistants on the hall as needed.

Must maintain a courteous and professional manner with residents, visitors, and coworkers.

Must be willing to comply with institutional regulations, policies, and procedures.

Good physical health and mental alertness are required to perform job functions efficiently.

Must possess the ability to accept and follow written and verbal directives.

Must utilize good communication and observation skills.
