Job summary

Alumus is seeking a Licensed Practical Nurse (LPN/LVN) to provide high-quality in-home nursing care, applying the nursing process to ensure continuity and safety for patients in their residences.

The role requires a current LPN/LVN license and at least 2 years of experience, with the ability to educate patients and families about diseases, medications, and wound care while coordinating with the nursing team and physicians to maintain excellent care standards.

Qualifications

• Current, unencumbered LPN/LVN license required or Compact State license.
• A minimum of 2 years LPN/LVN experience.
• Ability to educate patients and family on multiple disease processes with strong medication knowledge and wound care experience.

Responsibilities

• Complete and scan required record keeping forms/charts upon the guest's admission, transfer, and/or discharge.
• Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the guest, as well as the guest's response to the care.
• Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Director of Nursing.
• Ensure that appropriate documentation concerning unauthorized discharges is entered in the guest's medical record in accordance with established procedures.
