Job summary

Santè is seeking a Licensed Practical Nurse to provide patient-centered nursing care in the comfort of the guest's residence, using the nursing process to ensure quality and continuity of care.

Qualified candidates hold a current LPN/LVN license (or Compact State license) with at least 2 years of experience and can educate patients and families on disease processes and medications. Competitive benefits and flexible scheduling support a rewarding career.

Qualifications

• Current, unencumbered LPN/LVN license required or Compact State license.
• A minimum of 2 years LPN/LVN experience required.
• Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.

Responsibilities

• Complete and scan required record keeping forms/charts upon the guest’s admission, transfer, and/or discharge.
• Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the guest, as well as the guest’s response to the care.
• Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the Director of Nursing.
• Ensure that appropriate documentation concerning unauthorized discharges is entered in the guest’s medical record in accordance with established procedures.
