Job summary

Sevita — ResCare Community Living in Texas is seeking an LPN/LVN to deliver nursing services, training, and supports for individuals with disabilities within a community setting.

You will coordinate care, monitor health, administer medications, and assist with ISP-driven plans, including staff training and liaising with external providers. Full, part-time, and on-call schedules are available.

Qualifications

• Associate's Degree in Nursing or related field required.
• One year of experience working with the population served in a nursing capacity.
• Current state LPN or LVN licensure for state of residence.
• Current driver's license, car registration, and auto insurance.
• Certifications such as CPR/FA, HIV/AIDS, BBP, and ACT training required by state licensing regulations.

Responsibilities

• Monitor the health conditions of individuals served and ensure quality services.
• Administer medications and provide documentation; oversee staff who administer medications.
• Provide prescribed medical treatment by physician's orders and personal care services.
• Conduct assessments and implement health care services in each ISP; respond to condition changes.
• Provide staff training on universal precautions, infection control, and medication administration.
• Liaise with medical and professional providers and coordinate services with external agencies.
