Job summary

Heart to Heart Hospice is seeking a Licensed Vocational Nurse/Licensed Practical Nurse to support patient care in a hospice setting. The role involves assisting the RN with the plan of care, ensuring timely documentation, and promoting patient self-care in collaboration with the care team.

Three 12-hour days per week, including every weekend, with full-time benefits. A current LVN/LPN license and a valid driver's license are required, plus reliable transportation and insurance coverage.

Qualifications

• Current Vocational Nurse or Practical Nurse license
• Graduate of an accredited school of vocational nursing
• Current Driver's License and reliable transportation
• Two years of clinical LVN/LPN experience preferred, hospice/home health preferred

Responsibilities

• Assist the RN in carrying out the plan of care.
• Supplement nursing care needs as per POC under RN direction.
• Help patient learn appropriate self-care techniques.
• Complete clinical and progress notes documenting outcomes.
• Assess patient/family response and communicate with team.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with laws, regulations, and policies.

Licensed Vocational Nurse/Licensed Practical Nurse

What You Must Have:

• Current Vocational Nurse State Licenseor Practical Nurse License
• Graduate of an accredited school of vocational nursing
• Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
• Two years of experience as a Licensed Vocational Nurse or Licensed Practical Nurse in a clinical care setting preferred, hospice or home health preferred

What You Will Do

• Assist the RN in carrying out the plan of care.
• Under the direction of the RN, supplements the nursing care needs of the patient as provided by the RN and as specified in the POC.
• Assist the patient in learning appropriate self-care techniques.
• Complete appropriate clinical and progress notes, documenting outcomes of interventions.
• Assess patient/family response to care, ensuring communication of information to appropriate team members.
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
