Job summary

Clear Creek Care Center in Westminster, CO is seeking a licensed practical nurse (LPN) to provide direct care and support to residents under physician orders. You will work in a collaborative team, supervise nursing staff, and ensure compliance with all standards of care while delivering high-quality, compassionate care.

The role requires an active CO LPN license, CPR certification, and preferred 1 year of long-term care experience.

Qualifications

• Graduate of an accredited school of Nursing.
• Current Colorado LPN license.
• CPR certification.
• Knowledge of Point Click Care and nursing procedures.
• One year of long-term care experience preferred.

Responsibilities

• Assess patients by physical examination and tests to determine health status.
• Administer medications and treatments as ordered.
• Participate in care planning processes.
• Supervise LPNs and certified nursing assistants.
• Communicate changes in residents' conditions with physicians.
• Document assessments and care per standards and policy.
• Educate patients and families on health-related issues.
• Complete forms and documents per company policy and regulations.
