The Long Term Care LPN works collaboratively with CNAs, providers, and the interdisciplinary care team to support consistent, resident-centered care and continuity across shifts. Through professional clinical practice, accurate documentation, compassionate engagement, and strong teamwork, this position helps maintain a supportive, therapeutic environment within Trinity Homes.

Key Responsibilities

• Assist the RN with assessments, data collection, and continuous monitoring of residents’ physical, emotional, and functional status.
• Provide delegated nursing care, including medication administration, treatments, wound care, and chronic condition monitoring in alignment with resident care plans.
• Document all assessments, interventions, and resident responses clearly and accurately in the electronic record to support regulatory compliance and continuity of care.
• Communicate promptly with the RN, provider, and family regarding changes in resident condition and assist with updates to individualized care plans.
• Supervise CNA staff during daily care activities, ensuring proper techniques, safety practices, and resident dignity are upheld.
• Support a safe, clean, and homelike environment by following infection prevention, safety, and equipment handling procedures.
