Job summary

Morristown Post Acute Rehabilitation and Nursing Center is seeking an LPN to join our weekend team. You will provide direct patient care, administer medications, monitor vital signs, and support ADLs for residents.

In this full-time weekend role, you will collaborate with a multidisciplinary team, develop care plans, document accurately, and contribute to quality improvement initiatives in a compassionate, patient-centered environment.

Qualifications

• Active LPN license in New Jersey (NJ).
• Prior experience in long-term care or skilled nursing preferred.
• Able to work weekend shifts (Sat and Sun).

Responsibilities

• Provide direct patient care, including medications and ADLs.
• Assist in developing and implementing individualized care plans.
• Perform patient assessments and monitor vital signs.
• Coordinate with physicians and the care team to ensure seamless patient care.
• Document patient data accurately and timely.
• Maintain a clean, safe clinical environment.
• Participate in quality improvement activities.
• Serve as a role model for other caregivers.

• Provide direct patient care, including administering medications, dressing wounds, and assisting with activities of daily living (ADLs)
• Work closely with the nursing team to develop and implement individualized care plans
• Conduct patient assessments, including vital signs and physical exams
• Collaborate with physicians and other healthcare professionals to coordinate patient care
• Document patient data accurately and in a timely manner
• Maintain a clean and organized clinical environment
• Participate in quality improvement initiatives and activities
• Act as a role model for other caregivers, demonstrating a commitment to delivering exceptional patient care
