Alumus Healthcare in Mesa, AZ is seeking a Licensed Practical Nurse to provide high‑quality nursing care to patients in the comfort of their residence, using the nursing process to support continuity of care.

Qualifications

• Current, unencumbered LPN/LVN license required.
• Minimum of 2 years LPN/LVN experience.
• Ability to educate patients and family on disease processes and medications.

Responsibilities

• Complete and scan required record keeping forms/charts upon the guest’s admission, transfer, and/or discharge.
• Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the guest, as well as the guest’s response to the care.
• Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the Director of Nursing.
• Ensure that appropriate documentation concerning unauthorized discharges is entered in the guest’s medical record in accordance with established procedures.

Licensed Practical Nurse
