Job summary

Adoration Health seeks a Private Duty LPN to join our Jasper, TN team for 1:1 patient care under a client’s plan of care. This role emphasizes compassionate, patient-centered support in a home-health setting, with a focus on independence and dignity.

The LPN will administer meds, provide ostomy care, document nursing actions, and coordinate supplies while collaborating with a care team. Must have an active LPN license and CPR certification.

Qualifications

• LPN/LPT/LVN Degree from an accredited college with current, unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• CPR professional certification.
• 1+ years of nursing experience (preferred).
• Home Health care experience (preferred).

Responsibilities

• Providing 1:1 care following client’s plan of care at all times while maintaining client dignity and independence.
• Administering medications, feedings, oxygen, and ostomy care as needed per client’s plan of care and provided assessments of results.
• Accurately documenting nursing actions of all care given and communicating with family, case manager, and physicians, as well as documenting progress and outcomes for established goals and informing stakeholders of changes in the client’s medical condition and needs.
• Coordinating or ordering supplies, medications, and feedings as needed to ensure adequate amounts are always on hand.
• Ensuring all patient needs are met in accordance with the instructions of the physician.
