Rutland Center for Living and Rehabilitation is seeking a compassionate LPN to join our per diem team, delivering patient-centered care across a premier long-term and subacute care setting. As an LPN Per Diem, you will administer medications, monitor residents, collaborate with RNs and other clinicians, and support quality care initiatives while maintaining HIPAA-compliant records.

Job Summary:

We are seeking a compassionate and experienced LPN to join our team at Rutland Center for Living and Rehabilitation, a premier residential, long-term, rehabilitation, and subacute care provider. As an LPN Per Diem, you will play a vital role in delivering exceptional patient care and supporting our mission to "Revolutionizing Care Delivery." As part of our per diem team, you will work on an as-needed basis, providing flexible and responsive care to our residents and patients.

Responsibilities:

• Provide high-quality, patient-centered care to residents and patients, including administering medications, wound care, and other treatments as needed

• Collaborate with interdisciplinary teams to develop and implement personalized care plans, ensuring coordinated and effective care delivery

• Conduct routine vital sign checks, monitor patient conditions, and report concerns to RNs or other healthcare professionals

• Participate in quality improvement initiatives and contribute to the development of best practices in patient care

• Maintain accurate and up-to-date patient records, adhering to HIPAA guidelines and facility policies

Requirements:

• Current LPN licensure in the state of Vermont

• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams

• Ability to work flexible hours, including evenings, weekends, and holidays, on an as-needed basis
