Luxe Home Health LLC seeks a compassionate Licensed Practical Nurse (LPN) to join our Indiana team on a per diem basis. This role focuses on delivering high-quality care to patients in their homes under the supervision of a registered nurse, with flexibility and meaningful patient impact.

Qualifikationen

As an LPN at Luxe Home Health, you will play an essential role in providing high-quality care to patients in their homes, working under the direction of a registered nurse. You will work with a supportive team to ensure patients' needs are met while maintaining the highest standards of care. Your responsibilities will include administering medications and treatments as prescribed by physicians, monitoring and assessing patients' conditions while reporting any changes to the clinical team, and providing education to patients and their families regarding care plans and health management. You will collaborate with other healthcare professionals to ensure comprehensive care and maintain accurate patient records and documentation according to policies and regulations.

Requirements for the LPN:
