Adoration Health is seeking a dedicated Home Health LPN to join our Charleston, WV team. The role focuses on providing compassionate, patient-centered care in a 1:1 home setting, following individualized plans of care with support from a clinical team.

The LPN/LPT/LVN license must be current in the state of practice; CPR certification is required; a valid driver’s license is essential for travel within the Kanawha County area.

Qualifikationen

• LPN/LPT/LVN Degree from an accredited nursing program with current license in practice state.
• CPR professional certification is required.
• Valid driver's license and proof of automobile insurance.
• 1+ years of nursing experience preferred.
• Home Health care experience preferred.

• Provide 1:1 patient care following the care plan and maintain patient dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed per plan.
• Document nursing actions and communicate with family, case manager, and physicians; report changes to the care team.

• Ensure patient needs are met per physician instructions.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
