Job summary

The Grand Rehabilitation and Nursing at River Valley is seeking an Evening LPN to join our team of healthcare professionals. You will provide high-quality patient care to residents in a warm and welcoming environment and support RNs with daily tasks during the 3-11 shift.

Responsibilities include direct patient care, ADLs, vital signs, medication administration, care plan implementation, and collaboration with the interdisciplinary team. We offer 1199 benefits and paid time off.

Qualifications

• Current/Active LPN licensure in New York.

• Ability to collaborate with RNs and interdisciplinary team.

• Collaborate with other healthcare professionals to ensure continuity of care and effective communication.
• Perform vital signs and other clinical tasks as needed.
• Maintain a safe, clean environment adhering to infection control standards.

Job Summary:

• Provide direct patient care to residents under the supervision of RNs including assistance with activities of daily living (ADLs), vital signs, and medication administration

• Participate in care conferences, including discharge planning and family meetings, to ensure comprehensive care and support

• Maintain accurate and detailed records of patient care, including medication administration, vital signs, and treatment plans

• Collaborate with other healthcare professionals to ensure continuity of care and effective communication

• Perform vital signs and other clinical tasks as needed

• Assist with maintaining a safe and clean environment, adhering to infection control standards
